Web form from Tracker URL

I have an email containing a tracker URL.

I would like it so that when the email link is clicked, you are directed to a new lead web form that can enter referral details.
When form is submitted, it will create the new lead but also fill in some fields with the name (or other details) of the lead / contact who originally received the email from the tracker URL.

Is that clear (confuses me!)

Hi David,

A before save logic hook on the interaction with the clicked link in the CRM could be used, to create a new record.
