I would like to create a web form page with custom fields such as eye color, hair color, etc which i can then view the results in SuiteCRM. Does SuiteCRM offer this capability, or is there any knowledge of a plugin from the SuiteCRM store you could recommend i use to accomplish this?

If you want to have simple field and related to Lead module. then you can do it via webtolead form in SuiteCRM. But if you wish to have the custom module as a form with HTML 5 support. then you can have a look this module WebtoModule
