Relationship field not working

i am on suitecrm 7.11.18

i have a built a relationship field for my custom modules and set up one to many relationship field
but the link field is not working and the link field is showing undefined error when i try to choose a record why is this happening

Have you tried doing Quick Repair and Rebuild??
