How to show a checkbox in PDF Template

  1. I created a custom PDF template for contact modules. I have also several custom checkboxes fields. How can I show them on PDF template as checkboxes.

  2. Is there any way to add some custom JavaScript to PDF template in an upgrade safe manner?
