How to create more than one controlbox and give a name to each box

Hello guys,
i am working on a project and i have a difficulty to create control boxes. Below is an image of the control box i mean

I want to create 3 of those boxes and name each of them,with the possibility of choosing all 3 boxes.
is this even possible?
If not ,is there an alternative?
