How to add sub modules under modules in create report

In the attached image, right now I have Cases Modules and on Create Report page I can see sub module under Cases Modules.

Similarly, I want to add Cases, Accounts as sub modules under Contact Module.
Also, want to display replated Fields to add report filter.

For Example:
Contact
---- --Cases
----- -Accounts
