It could be that the jQuery is not loading correctly that triggers/positions this element.
I ran the quick repair/rebuild.
is there something else to do?

Looks like a memcache issue, disabled it and now things are way faster and no more wondering icons.
My Icon is still wondering on the dashboard page…
Looks like an issue when copying the entire /themes directory to /custom/themes.
instead I replicated the folder structure and added the specific items I wanted custom, working now
