Custom Module: Contact as Line Items

Hello Suitecrm community,

Is there away to build a custom module contains Contact fields same as Email Address field with checkboxes and the Add and Remove buttons like the screenshoot belows?

Hi, I want to customize the checkboxes, the Opted Out and Invalid, is there any way to do that without coding or losing the updates if I update the CRM?

It’s possible without lost the updates but it isn’t without coding.
Which version of SuiteCRM do you use?
