Bugs Screen - Create New from Account not working

Guys,
Sorry long story. Converted from SugarCRM and that went well.
Bugs has been highly configured and is a main module for them almost like Case
Bugs Full form has 6 Relationship links and many fields.

Create works from module list
Create works from open Bug record.
Create does NOT work from the Account Sub Panel?
Create does NOT work from the Case Sub Panel

Any thoughts please… My guess is when in a sub panel it has troubles with too many relationship links ?

Is there actually a create link? If not, you would need to add it to the subpaneldefinitions.php for Bugs and perform a Quick Repair & Rebuild.
