It looks like you are running SuiteCRM 7.0.x.
Firstly, I would highly recommend you upgrade. The Workflow module alone has gone from version 1.0.x to version 2.0.x, amongst a vast array of bug fixes, new additions, enhancements and more.
See the release notes for all versions here.
If you need to run a workflow on any new leads, you do not need to add any conditions. Simply set the workflow to run on the Leads module and add actions as you have in your action panel.

Try changing condition to Date Created - Equal To - Any Change, not using it for leads but Cases and should work the same
