Open HTML using entryPoint and then point back to original page

Hi everyone,

I want to create a module, whose only purpose would be to open up HTML page to input some data and submit it.

I am able to redirect to this custom HTML page using the following link, but unable to get back to the home page, (or any custom page on CRM will work too).
