AOP File attachment empty

Hi,
I setup Suitecrm on local server in a customer with AOP
I can use the portal as user after created an acccount.
But when I create a ticket on the portal http://www.crmcaraibes.com/log if I attach a file with the message, no files appears on the portal and in the CRM the case is well created, contains the name of the attachment, linked with the ticket but does not contains any file.
I thought it was due to rights concerning folders mais it seems to be ok. In log, I saw only this fatal error concerning case :
[FATAL] Query Failed: INSERT INTO cases_cstm (id_c ,jjwg_maps_lat_c ,jjwg_maps_lng_c ) VALUES (β€˜a16b49ce-4ac0-0faf-f585-58cbffa7be02’ ,0 ,0 ): MySQL error 1062: Duplicate entry β€˜a16b49ce-4ac0-0faf-f585-58cbffa7be02’ for key β€˜PRIMARY’

In comparaison, I tried with another instance, setup exactly same things in suitecrm ondemand and all worked perfectly.

If somebody could help me to solve this problem or give me ideas, I would be very grateful !
